Register for the 2008-09 Season
Please complete the following steps to complete the registration process. Please print off and fill out the forms and then turn them in to Kristi Letourneau. You may contact her via email at: klet@bvchea.org
- Complete the family information sheet – and submit a $20.00 registration fee per player.
- Complete a player information/medical release for each player
- If you have not renewed your BVCHEA membership for 2008-09, please complete the membership application form and attach your $20.00 check. (Dues are $20.00 per family).
- Complete the Lincoln Center membership. One form per player. Fees are $25.00 per players, with a $75.00 maximum per family.
- Review the What Parents need to know document. Please review it, but you do not need to turn in a copy.
